Transitions, a free support program through a journey of change…

Adapting to the changes that accompany a life-limiting illness can be difficult, frustrating and sometimes confusing.  It can be challenging to keep one’s independence while also learning to rely more on others.  Our Transitions program offers support, education and practical assistance to anyone who may still be seeking curative treatment for their disease and may not be ready to make the decision to enter a hospice program.
