The life expectancy of a hospice patient can be months, weeks or days.  When a patient or family member chooses to bring in a hospice team they will find a variety of services offered, not only for the patient but for those who will be left behind.

Centered in a holistic approach to care, hospice integrates medical, emotional and social services, as well as spiritual support, to meet the individual needs of each patient and their families. Working with the patient and family, a care plan is tailored to the individual patient and family needs.

An interdisciplinary team usually includes:

  • The patient’s Primary Care Physician and the Hospice Medical Director
  • Registered Nurses
  • Social Service Counselors, Grief Counselors and a Chaplain for spiritual support
  • Home Health Aides or Certified Nursing Assistants to provide personal care
  • Trained Volunteers
  • Complimentary therapies that may help relieve symptoms such as massage, music and acupuncture therapy

The Medicare Hospice Benefit also provides for:

  • Medical and nursing care, with routine home visits by registered nurses
  • Medical equipment and supplies
  • Medications for symptom management and pain relief associated with the diagnosis
  • Short-term inpatient and respite care
  • Physical, speech and occupational therapy
  • Homemaker and home health aide services
  • Bereavement and other counseling services
  • Social services
  • Spiritual counseling, bereavement services, and trained volunteer support
  • Grief support

Services are brought to the location of the patient, wherever they may call “home.”
