Typically, a family member serves as the primary caregiver for the hospice patient in their home. Members of the hospice care team make regular visits to help families care for their loved one through their end-of-life journey. They assess the patient's needs and provide additional care if necessary, answer questions, make changes when necessary to control pain and symptoms and continually provide support to the family. |

Patients with multiple life-limiting illnesses are treated no differently than one with a single illness. For example, a patient may have a life-limiting heart condition and cancer. HospiceCenter will treat both. Medically unrelated problems, however, would be reviewed and cared for separately. Team members are available 24 hours a day to answer questions or provide additional assistance or care. |
